Sick Chicken - breathing heavily, lethargic

1) What type of bird, age and weight (does the chicken seem or feel lighter or thinner than the others.) - Breed is Dominique. Top chicken in pecking order. Don't know if she's small for her breed. She's one of the two largest of our 5 hens. Age is approximately 5 months. May have just started laying (we got our first egg yesterday! Not sure who it was by).
2) What is the behavior, exactly. This morning, she did not come out of the coop - she was still on the roosting bar. She was sitting on it, with her eyes closed and her mouth open. I just moved her out of the roost area because it gets hot during the day to a shaded area. She didn't try to avoid me picking her up at all, which she would do if she was healthy. She just settled down, and has not moved about since. She is standing there, breathing heavily, either staring straight ahead, or closing her eyes.
3) How long has the bird been exhibiting symptoms?Yesterday maybe she started, but not since. About a month ago, she was sick and we gave her fish antibiotics.
4) Are other birds exhibiting the same symptoms? Not at this time.
5) Is there any bleeding, injury, broken bones or other sign of trauma. Not at this time
6) What happened, if anything that you know of, that may have caused the situation. It's been hotter than normal here, up into the 80's. We've been giving our squad ice cubes and ice blocks and have set up more shade.
7) What has the bird been eating and drinking, if at all. Since this morning, she hasn't eaten anything. I just dripped a bit of water into her mouth. Hope I didn't choke her!
8) How does the poop look? Normal? Bloody? Runny? etc. Poop is runny, both green and white. Not bloody for the sample I'm pretty sure is hers.
9) What has been the treatment you have administered so far? Nothing so far, just moved her out of the hot roosting box/coop and into a shady spot.
10 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet? Our plan is to treat her ourselves if we can, although maybe we'll take her to a vet if she's really bad. We are probably going to try to give her the antibiotics again this evening - once my wife comes home from work.
11) If you have a picture of the wound or condition, please post it. It may help. See below. Also took a video, but not sure how to get that from my phone to this forum (I don't have a browser on my phone)
12) Describe the housing/bedding in use. The coop area is smallish, we only have 5 hens. The bedding we only use in the nesting boxes, and it's shredded up wood. You can see a bit of it in the picture.

She probably has gape worm I don't know much about treatment except to isolate her. If you search it you will probably find more info on google. I hope this helps

Poop is runny, both green and white.
She was sick a month ago - what were her symptoms?
Have you looked inside her beak for obstructions or lesions?
Any coughing, sneezing, etc?

Dear friend, please do not toss antibiotics at an unidentified illness as it could mask symptoms or make others worse.

First things to check are her crop function is it sour or blocked? Is it puffy, empty, hard, etc? And droppings.. Is she passing any, what color, consistency?

I checked her crop. Seems like it's puffy. Unfortunately, I couldn't really find it. I tried to find similar on a couple of my other chickens and if I found them, they weren't firm. If her breath was sour, I didn't smell it from about 6" away.

I'll go in a bit to look for obstructions or lesions.
I'll take a photo of poop I think is hers from last night.

No coughing or sneezing, at least that I can recognize.

Sick a month ago - she was standing still, and her head was drooping down and her eyes were closed, and her eyes also got cloudy. Today the eye thing isn't happening, nor was the drooping. We asked our local "chicken lady" who suggested the antibiotics, and that worked.
