Sigh, I'm rehoming my dog tomorrow, how does this sound?

I'm rehoming my Newfoundland dog tomorrow

After a year of mulling it over and going back and forth I have finally decided once and for all that I am going to adopt him out. I just am not at a point in my life where I can devote the time to him that I was able to when I finally got him after 4 1/2 years for him and spending around $3500 for him...
He deserves a better owner than I can be right now and a better fit of a home. He wants to be outside all the time and when he can't be he gets depressed and loses weight. I have had a rough year medically and have had a good amount of health problems and other things and just cannot manage a 200 pound dog on my own when my husband isn't here. He can't help being rowdy, he's young and wants to play. He has some training but also KNOWS he's big... I just failed all around with him.
I had waited and dreamed and wished for him. I had planned to show and breed Newfs. Then my life went upside down and everything changed 2 months after I finally brought him home. He's now about to turn 2, so even though my life changed drastically, I held on for over a year trying to make it work with him.
Now i've gotten to the point where he's not happy and I can see it. He deserves better. I found him a loving family also on a farm who has a huge fenced in back yard for him. He's leaving tomorrow. Tonight he gets all groomed out and bathed and dried and treated with flea and tick and heartworm (usually the 1rst of the month for heartworm but I want to treat him before he leaves in case they need a few days to get him to the vet).
Anyway, I just wrote up the adoption agreement, how does it sound?

On this day the 31rst of August, 2009 we Jenna and Keith _________ turn over ownership of one Bronze male Newfoundland Dog named Gryffin to Cheryl and Eddie _______________. This dog was whelped on September 23, 2007 and is 23 months old. This dog is an intact male. The prior owner knows of no health problems in this dog at the time of sale and this dog was last treated with topical flea preventive on August 30, 2009 and Interceptor Heartworm preventive on August 30, 2009.
By signing this paper and removing said dog from the property of Jenna and Keith the new owners are responsible for this dogs mental and physical well being and agree to keep in touch periodically with the prior owners and to contact them at any time that they find themselves no longer able to care for him BEFORE adopting or surrendering him to any other private person or adoption facility.
Furthermore, the new owners are financially responsible for any and all veterinary expenses from the time of signing this paper onward.
