Looks like your chicken it just trying to cool off. I wouldn't worry if she is walking fine, eating and drinking. Keep an eye on her poops. Also when was the last time you dewormed your flock?

Ah I see. That's not too bad!

Chickens laying on their side can be quite normal. They do it to sun bathe, dust bathe, cool off, etc.:)

Is this something she has always done or just recently?

She does this a lot but then she gets up like when she was four or three weeks old she was lying on side kicking her legs up looking like she having a hard time to get up but then she gets up and is fine it always make me feel freaked out but then relief

I'd say seperate her and give her a full inspection. Could be early signs of paralysis etc.. in the pic you put the eye looks a bit odd. Can you send a pic of her eyes? Thanks.

well I am new to taking care of chickens and they are about 12 weeks old but how would you dewormed them?

You can use feed that can deworm your flock like flubenvet or other dewormers you can find (don't use natural ones like verm x it isn't effective)
