the jumping is actually easiest to fix, at least for most dogs.. It will require an assistant to proof the behavior but you can do the foundation yourself.
She simply gets no attention if she doesn't have all 4 feet on the floor. Greet her in an excited voice. If she starts to jump, simply turn your back. You may have to keep turning in a circle, face tilted up to avoid making eye contact, and arms crossed over your chest. The behavior will get worse the first few times because jumping has gotten her attention in the past.
The second she slows down, look at her and say "sit" when her butt is on the floor, she gets attention.
