Top of Chicken's head is ripped off

I agree, more photos may help. I'm having a hard time figuring out what I'm seeing.

The frank blood - could she have a broken feather that is bleeding?
The clots, I just don't know!

Where is she being kept? Is she indoors where nothing can get at her like a cat, rat, etc.?
She's been kept indoors so there are no animals that can get her.
 
How can I take a picture without the feathers?
It may help if you can get someone to help you part the feathers while you take the picture. A vent picture shouldn't be too bad to get a clear shot of, as long as you can get some decent lighting and keep the feathers out of the way. With the clotting on the leg I am not sure if you can get a better picture than the one you provided. It may help to try to clean up the area a bit so we can see what is going on there better but I would wait to do that until someone else gives their opinion. It could cause it to bleed more so I am not sure if that should be done or not.
 
I️ have a chicken that looked exactly like yours after being scalped by a turkey. I’ll post a few pics. Her whole comb was purplish black so we cut of the dead stuff and cauterized it. I️ used damp cotton swabs to clean off the blood around her ears, eyes, and nose. We kept her in a kennel in the house for a week then let her go outside, under supervision, away from the other birds. We kept bringing her in at night and she slept in the kennel. I️ put bag balm on it other first day but stopper putting it on so that it could scab over. Once it scabbed over, we knew she would start doing better. Eventually the scabs started falling off. She is now back with the rest of the birds but is scared to death of the turkeys. She hasn’t healed completely. She runs across our orchard in the middle of the day with half her skull exposed, but as long as the others leave her alone, she may as well be allowed to be happy in the sun instead of kept in a dark kennel all day.
 
These are the before she was injured, before she was cauterized, and after she was cauterized. This was around April 4, 2020. I’ll see if I️ have more recent pictures.
