Trying to mix Duramycin-10 with water Please Help

Hello, I am trying to get the measurements for water and Duramycin-10 if anyone has any suggestions. It says on the package to mix 400-800mg to one gallon of water. Any help on the amount of what would equal 400-800 mg's ?????? I am guessing close to 1/4 teaspoons per gallon of water. Am I close enough??
I really don't even need a gallon but just to make it easier I thought I would go ahead and mix it up this way. I am only going to give it to one 2 month old chick.
 
I have a scale and measured either 19 grams of power in a sandwich bag or .6 oz in a sandwich bag.
No dead chickens and didn't help, but we may have different illness'.

Forgot to add that this was for 3.5 gallons.

tablespoon per gallon. that's what i used~

it isn't the best stuff to use. What are you treating? I was told don't bother with the stuff. tylan and sumlet work great but for different things. I ditched it and got Tylan for the water 1/4 teaspoon per quart

I have 1 chick out of 14 that stopped eating and drinking. I posted info about her several days ago. I went back to the feed store where I bought them this morning and he said he had 1 chick doing the same thing and is giving them the dermycin. He also added he is no chicken expert but that just what he was giving the whole flock. I notice she was the only one that started ruffeling up her feathers and standing away from the other chicks. I have them on chick start with medication. She is a two month old Buff Orphanington. I also bought Sulmet, should I try this also. I have to feed her with a syringe and she will swallow but will not drink or eat on her own. I am giving her parkeet baby food mixed with chick start and warm water in a dropper, three or four times a day. Not nearly enough to keep her alive, but she is still hanging in there. I have her in the house in a dog kennel keeping a warm bean bag under a towel to keep her warm. Anything else I can try please let me know. I will sure give it a try. Also, trying to bake and get ready for tomorrow
