I had something kind of like this once. It wasn’t a full hard shelled egg, but it was a lash layer on the outside of a smallish soft shelled egg, the contents of which were otherwise normal. My hypothesis at the time was it probably happened because the egg probably got stuck for some time, since the hen in question got poisoned from eating a bad plant and had temporary paralysis; anything a ways up in the egg system would have been unable to pass due to muscle weakness that took several days to fully resolve. She did get treatment for possible infection due to the presence of lash material even if in an atypical place, but she recovered and lays normal eggs now.

It is an egg within an egg, a defect that rarely happens when the egg reverses direction as a second egg is being produced. If you do a google search for “egg within an egg phenomenon” online you will find a lot of videos. Sometimes they are covered with a shell, and sometimes just a memebrane.
