Do you see any maggots on the wound? Does it have a bad odor?
How is your hen acting, is she alert or lethargic?
Is she able to poop?
To me, that looks like Flystrike instead of prolapse. Flush/soak her with saline, chlorhexidine or betadine - rinse her really well. If you find maggots, get all those off.
Apply triple antibiotic ointment or vetericyn to the wound.

Thank you so much! I read your info & will start tomorrow am. I had rinsed her off, noticed the area & trimmed her feathers ASAP. I put witch hazel on her & then doused her w Neosporin... (one of the suggestions in the article says not to apply Neosporin bcz it creates a happy area for flies to lay eggs...?)
To answer your questions,
She is eating, drinking, pooping fine.
She has been acting fine, til yesterday when I noticed she distanced herself from the other girls.
She is about 7-8 years old & she was acting recently as if she were getting ascites, slowing down & resting a lot.... but I found this today....
I gave her a nice clean area to sleep tonight alone & will get a better look in the am.
