What is happening to my chicken?

If a hen has chronic crop issues, you need to take a look at the possibility of pendulous crop.

In the past, I've helped folks battle crop issues that just refuse to resolve. Once we strap on a crop bra so the crop is supported properly so the contents are lifted up high enough to reach the 'drain', the crop empties each night, and the yeasts cease to be a problem.
 
If she is having crop issues like this I would stop feeding everything but layer feed
Maybe get layer feed crumbles or smash your feed into crumbles
I could be wrong and someone correct me please if I am but if this were my hen I would mix small amount of grit into layer crumbles and give fresh water with electrolytes
Even unflavored Pedialyte
Bread, fruit, veggies, treats all that needs to stop due to needing grit to go down and she doesn't have a good
record of crop working properly
Look down her throat with a flashlight also in case there's inflammation or something
If you only feed everyone layer feed she could go back with flock
 
Shes quite fat still so I don't think Its affecting her nutrition.

When you say she is quite fat.... where do you mean?? Is her belly big... between her legs and below her vent? The crop may not be the origin of the problem but possibly a symptom of an issue lower down her digestive tract. Problems with the reproductive system can cause abdominal swelling that would make a hen appear fat. It also constricts the gut meaning that food moves more slowly through the digestive tract and that would manifest in the crop being slow. Another problem can be that the hen has developed too much fat and it has formed around the gut causing the same problem.

Unless you isolate this bird you are really not going to be able to assess and treat her condition appropriately.
