Why did she die?

I thank everyone for their kind words. I hope to keep this topic going not only for myself to prevent this again, but to others who may find anything helpful to keep this from happening to their own flock/babies.

My girls are a year old now. Over the past week this same hen had been laying her eggs not in the nest but along side hubs' workshop. He had collected all the eggs and I had put them in their own carton. Saturday I made deviled eggs from this same hen that passed away. I had noticed that her shells were getting softer and softer. I didn't realize that all the eggs were from the same hen until yesterday then remembered the soft shell issue. Could this be related? Why are the shells softer? They are totally free range. All the other girls have usual eggs, but this one girl was getting softer. Was this the sign I was looking for? Is a soft shell a sign of something other than lack of calcium? I feed layer pellets, scratch grains, wild bird seed, and many other extras including meat, bread, and sometimes leftover pizza. Is the pizza treat a definite no-no? I will stop as of yesterday if this is so. I am familiar with rodents and fatty liver from eating too much BOSS.

We do have an Avian/Exotic vet in Indianapolis near 71st and Michigan. I also have a few farm vets around. I'll start making calls now so I can see who does what and where to go in case of an emergency in the future although I hope to never need them again. Even $30 isn't bad for peace of mind.
