I would call CPS.The neighbor would blame you even if another neighbor calls.Who cares about that? If it is really that bad the boy will be removed into foster until the mother jumps through some CPS hoops to prove she is well enough to have her child.Ofcourse the boy COULD end up with granny.Not sure if children of that age even have a choice.

Even a call to the police might jump start things.They would see the inside and be required to report it.In the least it could be considered a fire hazard from what I have seen on those hoarder/buried alive shows.Usually a complaint called in about the home leads to mental health services and CPS if there is kids.

You will feel better if you report this.Might be to late to help the mom,but the boy still deserves a chance.Reminds me of the book *The boy called It*

If you're not comfortable calling CPS, how about the health dept? The amount of clutter/mess in the house will need a look at, and they can then involve CPS or not. You know the mother's screwed up, maybe knowing she's being checked up on will help calm her down. Either way if the kid was your nephew and the neighbor knew something was wrong, wouldn't you want them to call?
