12 Week old Chick sick for over a month now... PLEASE HELP!!!

Maybe I can help a little with this post. I had some young birds shipped in back in March and not long after they got to my home, they all got sick. I tried many different things and then got Tylan. This stuff is super. The breakdown that I used was 1 teaspoon to 5 gallons of water. I only use one gallon plastic fountains so it was a little hard to guess at 1/5 of a teaspoon. I used it for approx a week and my birds were all better. I did put a couple teaspoons of ACV in the water also. Seemed to do the trick for me. I used a powder from Tractor Supply with no help, used Sulmet it didn't seem to work either but the Tylan did. I think I got it from Jeffers Supply. I wish you the best. This was a very nerve racking time for me being these were the Rare Australian Light Susses variety that Greenfire Farms imported. They did not come directly from Greenfire Farms though. They had to pick something up during shipping. Good luck to you.
 
You could be mixing too strong a mixture of Sulmet. I read somplace that is should not be used on chicks, just older birds. I only use Corid for cocci now, the sulmet can cause internal bleeding from what I had read. I also had given it to a pair of auracanas that I had about 2 years ago. After I had them for about 4 weeks, I noticed some blood in the shavings and in the poo. I treated with the Sulmet and in a few days the poor birds were a bloody mess on their rear ends. They ended up dying and I think the Sulmet caused it.

The dosage I always used for the chickens is 2 tbsp of ACV to gallon of water, and NO, do not use it with anything else in the water such as vitamins or medications, and put it in plastic or glass waterers only. You don't want to put it in metal waterers due to the acidity.
The acidity in the vinegar can corrode metal so it can contaminate the water. There is a great article in backyard poultry april/may 2008 on the benefits of apple cider vinegar.
Unpastuerized ACV is a form of probiotics which can boost an animal's immune system. Strong immune systems are more resistant to the affects of cocci. Probiotics maintain a normal and healthy level of culture~good bacteria~in the bowels which prevents the more harmful, but naturally occuring, cocci bacteria from proliferating to the degree that it causes diarrhea, ulcerated bowels, dehydration, etc.
I buy the raw unfiltered ACV with the "Mother". I use it in my chickens water it helps keep the green slime from building up in the plastic water bowls and it is very beneficial to them. I was told by a breeder to add ACV to water not only to help with parasites, but to keep the water slime free in the hot summer weather.
