Sorry about your baby...a chicks development is so fast over such a short period of time. A slight change in temp at the wrong time can change everything. In people there is something called fetus infetu where one child in a set of twins develops around around the other. Its okay for a while but then the born child, at about elementary school age, looks pregnant. Not too many cases. Such is the case in animals too (hence the cow with extra legs). Your chick could have been a double yoker and that 3rd leg may have belonged to her twin. What you saw that burst could have been what was left undeveloped. Her little body probably couldn't handle the extra strain.

Definitly call McMurray. They don't come across as shady people to me and may be able to shed some light on your little girls condition.
