I always understood it as being how many germs that will cause problems for people, not how many germs total

IDK but when something will eat $h!t, puke and half rotted caucuses it doesn't sound like it would have a cleaner mouth tbey a humans whether your talking total bacteria or harmful to humans bacteria.
