Did you bottle come with the syringe?
For chickens and ducks: 0.2-0.4 mg/kg PO, SC, IM, topical once, and can be repeated in 10-14 days.

Now i have read 4-5 drops between the wing on the back for large birds, 2-3 drops for bantam.
I was told by a vet that it doesn't matter. you can inject, oral or topical. 1% injectable ivermectin is absorbed thought the skin. but withhold egg for 7 days minimum.

No it did not come with anything else. Just the bottle.
 
Thank you for the photo, that is 1% so the dose would be as posted.
1% Ivermectin injectable dose for poultry is 1mg/kg or 0.045ml per pound of weight. (0.23ml for a 5lb bird). Injectable can be given orally, topically or by injection. Dose once, then repeat in 10-14 days.

Bimectrin has 10mg of Ivermectrin per ML, so that would be the 1% solution.
