Hello. I have a few suggestions even though I am still learning myself, I have learned a lot through many ugly but tasty loaves.
I did not think that flour brand matters but it really does. I use King Arthur unbleached all purpose flour. I weigh my ingredients with a kitchen scale because I realized that I tend to pack my ingredients when I scoop it to measuring cups.
I noticed 3 cuts, try 4. Are you doing the cuts at a 45 degree? Splits happen cause the crust forming slower than the expansion of the loaf. Try spraying you loaf with warm water or use a cast iron pan in the oven heated and pour water in it to make your oven steamy.
