Both yes. I read that’s it’s harmful for their lungs to have the cover on, that true? Although it has ventilation on the top.
It could be harmful if it wasn't cleaned regular enough and had a lot of ammonia built up.
I don't keep a lid on my box because I wanted to make it as easy as possible for my rabbit to use it.

I think that maybe taking the cover off would be better until they knew that was the only place to potty.
I am wondering if they might be afraid of the door... And only go in there when they're absolutely starving.

I'm not sure I'm just thinking out loud.

You should at least remove the door from it. Some rabbits prefer the lid, whereas others won't use one with it.
I would remove both and see if they use it more.

Are you putting any poop that's outside the litter box back into it? Urine should be deodorized too if it's outside the box.
 
They really do like to hide in it though. 🤷🏼‍♀️ And i clean it at least 2 times a week.
If they actually go in there on their own then maybe if you just move the rest of their food into there where they don't have an option to eat anywhere but in there then maybe they'll learn to poop only in there.

They eat a lot of hay every day and seem to know how to use it very well. They just push it with they’re nose.
