Chicken has serious rear end issues

I notice this morning that one of my hens back end was wet and what I thought was poopy. Turns out her vent in outside of her, she is dripping some sort of liquid and what I thought was poop stuck to her vent area appears to be attached to her. I cleaned her up and put Vaseline on her vent area (that is what some of you suggested ) and separated her from the others. It looks incredabliy sore and I did not want my rooster mounting the poor thing.

Does anyone have any idea what is wrong with her and what I should do to treat her? Should I keep her seperated from the others. Any help would be very much appreciated.

Thank you Babs! That looks exactly like what I am dealing with. I took as much of the old poopy that was stuck to her vent off, and I think it was quite painful for her but I got it as clean as I could. I did not have this spay but cleansed her with warm water. I then pushed her vent back were it should be and put Perpetration H around her vent. She is now resting in a dark spare room of oue house where I can keep an eye on her. I once again am so grateful for the information as I don't know what I would do if I didn't have all you guys to help educate me.
