Chicken losing appetite and weird poo(caused by worms?)

My 5 year old Australorp is acting weird lately. She hasn't laid an egg for around a week, her appetite has decreased by a lot, her crop is very soft and small when
its nighttime. She rests more often than normal, she still forages, drinks but at a slower pace. She also poops out weird coloured poops(picture) should they
