This is just a helpful hint for keeping your flock healthy.
Copper sulfate is a natural resource that you can buy. It can be given to pretty much all animals and is very useful for maintaining a healthy flock. To give the chickens copper sulfate, mix a quarter teaspoon of it in a one gallon chicken waterer. It will kill parasites and overall keep your chickens healthy.

Supplier? Brand? Length and Timing for Dosing?

You give your chickens CS 2 times a week for about a month.The supplier is Lancaster Ag. Not sure what the brand is.
 
Is there any set time frame they should be given it? Ex. 2x's week for a month every 6 months, every year or just when notice possible problems?
 
Just when you think your chickens need it, like when they have feather loss, enteritis, and other mild illnesses.
 
Just a suggestion, but you might want to take care with dosages and handling of copper sulfate (CuSO4) - it's pretty toxic stuff. It's very popular as a fungicide and as an algicide. Eye and skin contact are advised against. When you buy it, there should be a safety sheet (MSDS) with info on handling and first aid.

i was surprised to find it used for chickens as it is what we use for algae control in our pool lol.
