My left index finger is my yarn tension finger. I have a permanent crease between the top and middle joints from the yarn moving across it for years.I think I have to take a break from crocheting!My arm/wrist are aching and I haven't touched a hook for 2 days.

I had a callous of sorts on my left index finger for awhile from "scooping" the yarn with a knitting needle across that spot. Man, I really noticed when I went from a size 5 needle to a size 2!
