I don't deworm on a regular basis, some do, but I find it unnecessary and possibly even quite harmful. I have only ever dewormed once, and that was for tapes. Deworming on a regular basis allows the worms to become resistant to the wormer, which is not good.

Twice a year as in two doses, counts as one deworming. I usually deworm once a year (2 doses), but this year I will deworm twice.
Most commonly, my hens get roundworms and the thing about most worms is that they dont show up in their poop unless they have been there for awhile. Thats why I deworm yearly to be safe. If a hen has a huge infestation she could die.
I am in NJ - I give them kitchen scraps like tomatoes, cucumbers and veggies also bits of melon or water melon.

Late summer and late or mid winter (usually when the hens stop laying). There is an egg withdrawel period, so I usually only deworm once (2 doses), when the hens stop laying.
If I only deworm once (2 doses), I deworm during mid winter when I dont get eggs.
