Fish Mox for Salpingitis dosage and administration

Hello fellow flock keepers. I have a 2-3 year old hen who has laid 2 lash eggs. She is isolated and after 3 days of Poultry Cell is eating and drinking again. I just ordered some Fish Mox 500 mg and was wondering if someone can tell me how to use it. I am assuming I mix it with water. My question is how much water do I mix the capsule with? How much of that mixture do I give her? How often? For how long?

I have been reading a lot about it and I know its a long shot, but I figure it is worth a try.
