HELP CHICKEN GOT ATTACKED BY DOG

I'll get a video in the morning. She is walking a little but not as much as she would before the injury. She isn't really eating a ton on her own. I still have to feed her. Should I be concerned?
Yes, I would be concerned since it's been over a month since she was injured.
Has her neck straightened out?
Maybe take her outside for short periods of time as well if you haven't done that - a little sunshine and sit in the grass. Sometimes they can be a bit depressed.

her neck has straightened out a bit. she moves it around in different positions and holds it normally. i'll bring her outside more
 
Also, sunshine is how they get vitamin D into their little bodies. If she hasn't been outside much, she may need that vitamin boost.

And in case you haven't seen it happen before, when in jusssst the right light & in jusssst the right position, a chicken will get this glassed-over look in their eyes, stiffen up, kinda fall onto one side, stretch out a wing, maybe even fluff out their feathers & lie there soaking up the sunshine & vitamin D. Just go with it & stand guard quietly until she pops out of her trance.

It's heart stopping if you're not expecting it & don't know that's a thing, then hilarious every time after that. Morning sun, afternoon sun, evening sun....I've seen it happen at different times during the day.

Had a beautiful Barred Rock roo once who puffed himself out like a Lionfish flopped over & stayed like that throughout his sun bake, feathers all standing on end. He was magnificent anyway & that was the most incredible & interesting thing I ever saw him do...and he was a very sweet & amazing gentleman, so that's saying something!
 
i've been bringing her outside but today she was acting kindof weird. i don't know if she got too cold in our house because we have AC on or if something is really wrong. i found her in this position today and this morning she was sitting up right using her legs to push her butt in the air. but now its in the new position. i'll attach a photo. her feathers are also ruffled.

She could be trying to balance inside and/or trying to absorb vitamin D from her lights inside, too.

Once, I had a chicken try to sun bake in the glow from her heat lamp bulb while streching precariously on her perch! Their brains think "Light!! Cool, I'll have some of that, please!" They don't quite get that it's not sunlight.

All fluffed up like that is exactly what my rooster looked like! I think she's fine, just doing her chicken thing.

Worry is normal and you don't realize how invested you are in their health & comfort until you exhale. You're doing a good job & she's responded well to all of your tending. I'm sure that she will continue to improve.
