Big Blue Hen is right that is bruising. Edited to say, I think this is now possible necrotic skin from interrupted circulation (too tight dressing.) See post 12 below. I would flush the hole with saline or wound spray, apply antibiotic ointment to it and the red area on the toe. Then dress it with a 2x2 and wrap in strips of vet wrap or cohesive dressing.
