It might be yolk you're seeing... Another member here had a chick puncture the yolk save before it absorbed, but the chick was O.K. Look here:

What does the membrane look like? Is there any blood still? Have you looked at the assisted hatch article?
Antibacterial lotion, yes, but one without painkiller! Use a Q-tip and put it on as much of the area you can reach.
