Feel her abdomen. Is it hard like a water balloon filled with water, firm? IT looks like asceties to me- A buildup of fluid just below the vent/in the abdomen area. If she's walking like a penguin, chances are the swelling is either asceties or internal laying. If you can;t feel an egg and it's just firm, it's probably asceties.
If it's firm and it IS asceties, I'd recommend you go to your local tractor supply and purchase some syringes, as well as some 18 gauge needles. From there, you can drain the fluid from her- I wouldn;t recommend turning her on her back to get access to the belly. Instead, have another person hold her above you and access it that way. One of my hens had lung problems, and when I turned her on her back, it caused too much lung stress, and she died.
From here, sterilize a small patch of skin, as well as the needle. I'd insert the needle below the vent, angled downward. Pull back the plunger slowly- you should have yellowish brown fluid drain out into the syringe (sometimes with a little blood as well).
From there, empty the fluid into a can for disposal, and continue to drain. DON'T take too much fluid at once, or your girl could go into shock- also, some fluid may seep from the needle puncture. There may be some blood- if there is, hold a rag to the wound to slow the bleeding. I've had to do this procedure numerous times before, and while it isn't for the faint or heart, it helps the hen!
If you can't seem to draw any fluid, it's mostly likely internal laying- in that case, not much you can do accept a warm bath for her and prayers. 90% sure this is asceties, though- looks like the classic presentation that happens with my girls. Please update soon!
I generally don't with my girls- sounds like she's got quite a buildup, but I generally just sterilize the area with an alcohol wipe (an well as the needle) and make sure the area's clean once again before I put them back out. One needle prick won;t really put her at major risk for infection, and I wouldn't recommend using antibiotics, because they might contaminate her eggs or generally cause the bacteria in your flock to be come resistant due to exposure.
Asceties is generally caused by two things- congestive heart failure or liver problems. I'd suggest getting milk-thistle from your local health-foods store and giving it to your hen, as it should ease some of her liver symptoms (please do this at your own risk- while I don't think it would harm your hen at all, I have never personally given my hens milk thistle, so I can't vouch for it's effects. Just something I heard about).
It might also be helpful for you to google "Draining asceties fluid from chicken", as, though I've done it a few times, others might have tips or suggestions as well. Please tell me how she's doing!
