Hen cannot stay upright

hello all. I was given a chick with splayed leg syndrome. She is now about 6 months old and has been getting around good and has adapted. I had no idea I could have helped her out. I feel bad. Anyways yesterday I noticed she can no longer keep herself upright. If she is moving she can stay upright and she has moments that she will stay upright but spends most of the time layed out with her head on the ground. Her appetite is good and otherwise she seems fine. Any ideas of how to help her? Thank you.
