How did my chicken get hurt?

My chicken Caramel, was acting weird last Friday. Sadly, she died. But I would like to know why she started not moving a lot. It wasn't Lethargy because she still moved, but she walked with a limp, and her breast bone seemed broken? She wasn't eating much and barely could drink. She died yesterday at 7 am in my dad's hands. She had mucus in her feces, and before she died she had blood in it.

We tried everything but she still died, I think her breast bone broke and her lungs got hurt during it.
