How long has it been since the last? If she is not in active labor or distressed, then she is most likely just taking a nice long break.
Just to perhaps ease your mind a tad, mine went 4 and even 5 hours in between pups and they were all fine. And, this is probably a rarity, but my neighbours dog had a puppy almost a day later than the rest and it lived.

Well, *I* would not worry unless she is actively pushing without producing a puppy or seems in trouble. If she is just relaxing/resting than I would wait. What I did with mine was took a stethescope and listened (as it was a good 5 hours since the last) and I could hear another heart beat, so I knew that there was another in there. So perhaps you could try that if you have one. Another option is maybe just laying your hand on her stomach and feeling for kicks.
If your gut is telling you there is something wrong then I would call the vet.
As for the second labor question, I don't believe it is possible. I could be wrong, but considering there is only a short window of ovulation it wouldn't make sense to me. The eggs are ovulated, matured and if there is sperm present, they are fertilized and if not they die.
