My Chicken is dying and I'm not sure how

Do you think she could have eaten something poisonous or have any pesticides been used in your area that she may have accidentally injested? Maybe an Epsom salt flush would help. Does her crop feel empty or full? Does her beak look normal or like she has an overbite? How does she feel under her vent, is it squishy like a water Ballon? Does it feel hot to touch? Has she layed any eggs recently? How does her breast bone feel, is it sharp or soft?

I don't think she ate anything poisonous, But we did start them on something new, it's to help them lay eggs. She clicks her beak from time to time. The vent is squishy and her breast is soft. She hasn't layed eggs recently.

She is probably laying internally.
 
What did you feed them on a daily basis...compared to the new feed?
Chick feed/grower/layer/scratch?
Do you offer a complete chicken feed?
 
So sorry you are going through this. I have had a hen pass away from internal laying and it is really hard to watch. We kept ours as comfortable as possible for a while before she passed, but she was hanging with the flock and didn't seem to be in distress until she passed. She was doing the penguin walk for a while.
 
She can't walk, or stand, her legs give out. Her head kinds twitches/moves forwards and stuff when she's trying to look around. Her poop is kinda like broken egg, it's yellow white and green
We checked her vent for any signs of eggbound, but all that came out was this gross yellow foam like substance
She's maybe 4 years old,

How is your hen?
Can you post some photos of her poop and the yellow foamy substance?
How does her abdomen feel?

As suggested in earlier posts, she may have an internal laying/reproductive disorder like Egg Yolk Peritonitis. The head twitches are a bit different - sounds neurological. Check her ears for any mite/lice and infection. She hasn't fallen or been injured has she? If you have vet that can perform some testing on her poop that would be a good idea as well. Do your best to keep her hydrated, offer her some poultry vitamins and she if she will eat for you.

Here's a little more information on Egg Yolk Peritonitis:
From your description it sounds like she may have an internal laying/reproductive disorder like Egg Yolk Peritonitis, Ascites, cancer or tumors. Symptoms include lethargy, going off feed, swelling or fluid in the abdomen, penguin stance, lameness, difficulty breathing and sometimes runny poop that looks like it has egg in it.

UPDATE: Okay, my hen is doing well, I'm hoping this just gets out of her symptom, or just passes away peacefully. I've given her a tiny bit of antibiotic. Since then, she's had some water and struggled to eat. So tomorrow if she's alive, which I pray she will, I've softened and watered up her food to feed her it tomorrow morning.

I spread scratch feed on the ground and in their coop they have a few buckets of feed.
