My Chicken is dying and I'm not sure how

If antibiotics made a difference she may have some kind of infection I would continue the ab and see if she continues to improve. If so complete the regiment. Scratch may not have caused the problem, but you might want to look into a layer feed. Scratch is more of a treat or supplement.

Like I said before I have other feed I give them, plus they are free range they get worms and bugs too. So they do get a complete meal.
 
Update: Her poop is not watery anymore, which is good. I believe she might have ear mites. We have had mite problems ever since our neighbors bought us chickens because their dog killed almost our whole flock. I've given her a tiny bit of antibiotic. Along with having to kinda force feed her/ because she is still too weak to eat on her own. I've given her lots of water to try to kick whatever this is out of her. She's had lots of rest away from my other girls. And today we got her to stand!! I believe tomorrow I am going to clean up her butt because I'm worried about fly strike. She also has been very talkative, and the other hens have gone over to see her. So everything is looking good so far!

What antibiotic and dosage are you giving her?

For mites, use a Permethrin based poultry dust or spray. If they are in the ears, this could be the cause of the head twitching.
Having a fresh poop sample tested by your vet for worms would be a good idea as well.
 
Update! Well, she's starting to eat and drink on her own, but her coordination is really off. She'll drink the water and then move away from the water, and act like she's drinking water. The same goes with her food. I believe parasites are in her intestines, so I gave her some dewormer. She stood by herself today again!!

Which de-wormer did you give?

You mention she moves away from the water and acts like she is drinking - is it the dipping the head motion or does she look like she is swallowing?

Check her crop when she goes to bed, then first thing in the morning before she has had anything to eat/drink - I'm wondering if she is having trouble with her crop.
 
I'll check her crop in the morning before she eats. She might have fell and hit her head when I found her the very first day when I thought she was dying. Could she have brain trauma?
 
Update: She is doing wonderful, her poop is almost like normal. But something is still going on, she can't stand, yet she tries to occasionally. Nothing is stuck in her crop.
