I think perhaps it would be considered a 'Janus' chicken... aka diprosopus. Look up a Janus cat. Named for a Greek god with two faces. It seems to result from a miscoding in the DNA that controls the width of the fence, resulting in extra facial features. (The opposite creates the cyclops effect.) Thank you for being brave enough to share, I love to learn things, and I am sorry for you loss.

Fascinating. I would love to dissect it. I wonder if it had internal abnormalities, like two hearts, etc?

Well, I still have it because I was going to show it to my DH. I don't know if they would want it or not. Plus, I would not know how to contact them. Would 70% alcohol work?

Also put it in the fridge or it will decompose even in alcohol. Formaldehyde would be better.

Call the closest college, large high school or mortuary to get some formaldehyde to preserve it in a glass jar. It may be worth some money!
