As gross as it sounds, it does happen if the hen sticks too close to her eggs and doesn't come off the nest to eat, drink and poop. This usually happens in summer when there are lots of flies, but can pappen in the winter as well.
Take a clean box and fill it with clean nesting material and make a nest impression. With a pencil, mark the top of each egg. Gently brush off the eggs and place in the nest impression pencil mark up. Cover with a damp towel heated in your microwave for a couple of minutes. You might need to put several layers of papertowels over the eggs to keep a really hot towel from laying directly on the eggs.
Now clean your hen's bottom. After I bathe the butt, I dry her off, uncover the eggs and let her adjust in the nest.
