Permethrin Dilution - Updated with Product PDFs

Fixing to mix up some permethrin, looking at the charts...

Should these two parts of the 13% & 10% charts be switched.?

Permethrin comes in three different concentrations, 10% (most common, sold in most feed stores), 13.3% (common), and 36.8% (usually has to be ordered online). Below are tables that show how to make 1 gallon.

I'm using this one. We didn't see any pre-mixed permethrin for poultry at Tractor when we bought this, but we are only treating 12 chickens, so we don't need a gallon. We've treated them three times (weekly), and within a week, the mites / lice come back. (Not sure which it is, I made a thread on it here though.) We mixed the 1:199 ratio but we only did 1/4 of a gallon because we didn't need more than that. We mixed 4 cups of water to 4 and 3/4 ml of permethrin. Can anyone here double-check my math? I ask because we are about to spray the chickens for the fourth time and the infestation is bad. I want to kill these mites / lice once and for all!

I'm considering mixing enough this time to dip the birds, but I really wanted to avoid that mess. I know they will be splashing it everywhere, and I've never dipped a bird so I don't know how to properly do it. So I'd really like to spray them again with the proper dilution.
