What should I do? See the attached picture

she has been sitting in the awkward position for over a day now.

when she moves she really struggles to get on her feet - almost as if she is initially off balance.

she has eaten a little bit but I’ve had to put the food right in front of her.

Can she move her head up and down or around? She looks to be lame, and that can be due to a back injury, a spinal deformity that has pinched her nerves, or something like Mareks disease. How old is she? Was she vaccinated? Do you have a rooster who might have hurt her back? Can you get her drinking some water and give electrolytes or Poultry NutriDrench 2 ml daily. Offer moist chicken feed and scrambled egg, tuna, or canned cat food in moderation.

Chicken slings can be good to get a lame chicken up and kept clean, while placing food and water within reach. You also could place rolled towels around her to position her. Here is a link about how to make a chicken sling:

I have never seen this before. But, definitely isolate her. Do the basics; feed her yogurt, for natural probiotics. also, give her a warm bath. offer calcium supplements, like oyster shells. put her inside. if it is hot where you live, she may have had a heat stroke. make sure you offer other chickens shade and plenty of water. I'm sorry I can't help that much I've never seen this. When I have sick chickens I will research their symptoms, sometimes even on google scholar, and read scientific articles to help you diagnose her. OMG! I just remembered something. Check for a stuck egg! Sometimes when chickens have this they "hunch" over in a penguin-like stance. I would treat her for this. To do so, give her a warm bath. And, with a vaseline coated finger, put your finger in her vent a feel around d for a rounded mass. If you can feel the egg, pull it out. I like to give my chickens "human" antibiotics, dosed down for their tiny little bodies. I also feed mine yogurt, for the probiotics. And, ISOLATE HER! Try to minimize stress as much as you can! She may have Marek's disease, which means there is little to no chance of survival. And Marek's spreads, so definitely isolate her so your other chickens don't get sick. Marek's can cause paralysis.
