Fatty Liver Disease. As your report comments say - it's "believed" to be excessive dietary energy intake, but other factors such as genetic, environmental or hormonal factors may be involved.

So. Take that as a whole. To me, your feed is fine, but limit treats and if possible, let them get some exercise (roam about the yard).
 
Fatty liver hemorrhagic disease. I had one hen out of dozens that died an early death from that. It is sometimes related to a high fat and carb diet, but some hens are genetically predisposed, and some may get it from other reasons. That is why I do not give a lot of treats, scratch or sunflower seeds anymore. Sorry for your loss.

Yeah I'm definitely cutting out all treats except occasional veggies. I'm afraid for my other hens 🥺
