The few times it's happened to me I think all files were under 20 mp.

We needed to change from using GDlibrary for converting images to using ImageMagick

Curiously, did the problem have anything to do with the JPEG in queation having extra bytes? DH ran some sort of test on it and said that file had extra bytes.
