I’ve done epsom soaks, gently pushed it back in and it just keeps popping g back out. She’s uncomfortable and I’m feeling a bit helpless here. None of the vets around here will see chickens. Pic taken before her bath.

Can you get some photos after it's cleaned up?

Hard to tell, looks like damage with prolapse. The white looks like urates (poop).

How long has she been like this?

Keep the tissue moist with oil, ointment, honey, etc.
Thank you. I will clean her up again and get pics. She is a double yolk layer.Lays HUGE eggs... poor girlie. She has been like this three days. Today she is brighter and is out and about foraging, which is surprising to me. I keep putting it back in and it pops out with in a few minutes. There is no smell. She is eating and drinking normally but is quite skinny.

I agree it looks like the vent has been pecked, and there is a prolapsed vent. I would do some daily warm soaks to her bottom and apply ointment, honey, or cream to keep the prolapse moist. Try to push it back inside and hold it there for a few minutes.
