Sick chicken

My 1yo baby has her tail down, head turned sideways, lethargic, and won't get up. She looks like she's dying. Not sure what to do.

What is your location in the world? Could she have been injured or attacked? Does she lay eggs normally? It sounds like she could have wry neck (torticolis.) Wry neck is a neurological sign of head or neck injury, a vitamin E or thiamine deficiency, and is also seen in some diseases including Mareks disease, Newcastles, and some others. If you could post a picture of her, that might be helpful.
