Sick hen, paleness, lethargic and spot on tongue

I have never seen the type of red lesion on the tongue as your hen has. I would suspect a bite or peck from another chicken, but there could be some sort of cancer. Did you get human oral canker medicine, such as campho phenique, or did you get bird canker medicine such as Fish Zole. That does not look like bird canker, which is a protozoan infection. Those and wet fowl pox cause yellow thick caseous material in the beak.

I've never seen anything like that either. I would wonder if it's cancer. Interesting there's a scab/wound/lesion on either side of the nostrils too.

I would seek veterinary care if I were you. There is also this website called “justanswer” and it gets you in contact with professionals of all different categories, including bird veterinarians. Through that website I’ve been able to chat with a vet and send them pictures and they recommend treatments.
