Silkie doesn't seem right!

Hmm... this does take some figuring even after looking at the label on line. You may have already found the info, so this may just be a double check.

Here is what I found on the label give:
"10 mg/pound of body weight per day in divided doses."

Lets say she is 3lb so...
30 mg per day total thus two 15mg doses per day

I also found on the Durvet website: http://www.durvet.com/dl/Durvet-Poultry-Brochure.pdf
400mg dose is 1/2 teaspoon

That means 15mg is a really small amount. Let's assume that 1/4 teaspoon of powder is smallest amount to measure accurately. That would make 200mg.
Lets say you have a 10ml/cc syringe. Draw up 10 ml/cc of water, put it in a container and mix the 1/4 tsp powder, then draw up only .75cc of the solution (slightly less than 10%) That makes the 15mg dosage and you would give that twice a day. It must be mixed fresh every day, but you have way more powder than you need.

If you add the formula don't make it too thick, because it is harder to squeeze out of the syringe when thick.

I hope that all makes sense.

I did it! Not too bad to hold her in a towel. I did some electrolytes, antibiotic, and polivisol. Mixed with some water. Wasn't sure how much or how often, but got some in! I also put her in hrer own small cosy coop next to the roo.

That's great! I think when the label says "divided doses" that twice a day administration would work.
The 15 mg dosage is really tiny - like 2% of a half teaspoon. The only way I could figure to measure that accurately was to mix a measurable amount and then measure a small portion of the solution using the syringe. All of my syringes ml measure lines (1ml=1cc)
Just so you know 1ml = 0.20 teaspoon or one fifth of a teaspoon.

Thanks so much for your help! Update on her tonight. I have caught her eating her feed and I gave her some scrambled eggs and she ate them. Still not interested in drinking. Her balance is really bad and she gets around slowly. Most of the time has her neck pulled in resting and bobbing up and down. If I pick her up to snuggle she goes immediately limp and her eyes close and neck goes limp. When I put her down she has a hard time getting her feet under her. Poop looks normal although not a lot. Her eyes are closed and her head is bobbing around almost all the time. She is also missing a lot of feathers around her neck from the roo. I am mostly concerned now with her lack of balance and limpness.

I still need help. She has no balance at all and is always sleeping or sitting on her bum. Very off balance. Goes into a coma when I handle her to give her vitamins. She does eat like she is starved when encouraged to. Neck is constantly curled into a snug S shape, and she fights if I try to lift it up.

If you can post a photo of her with S shaped curled neck clearly featured that might help. The curl in her neck and the balance problem still sound like wry or crook neck to me, but you were not sure the description of the symptoms fit. Here is a link to another thread about a chicken with wry neck: https://www.backyardchickens.com/t/521570/polish-crested-wry-neck-crooked-neck-diary

You could try posting a link to this thread on the silkie thread to see if they could help with some advice: https://www.backyardchickens.com/t/297632/silkie-thread

Originally she was troubled with shaking her head and gaping her mouth. That went away. Now it is mostly balance and sleeping coma like when I am trying to feed her. I am going to try to upload a picture of her holding her neck. I am also trying to do some short video's to link to.
