Three of my birds were lost to drowning/hypothermia. Two were young and promising d’Anvers who roosted in the rafters and fell in the very huge buckets that were used to catch water (the shed had a very leaky roof until it was fixed.)
Then there was a Silkie hen from the roost in the coop. We have a hanging bucket that is usually covered as a nipple waterer. It was usually covered with the lid. That time it wasn’t. To be fair though, it was a Silkie. The other chickens could have flown out of that situation.

We always lose birds to the water troughs for the large animals. Not anything we can really do though that won't prevent the animals from drinking

Not underwater. Have it long enough it sticks out. I keep a big stick/2x4 in all my troughs.

That's what a rough stick is for. I typically just pickup a thick limb that's fallen and stick in them.
