Unsafe coop/entertainment examples

Hello! I am looking to see what people think are unsafe coop designs, builds or in-coop entertainment that would be potentially dangerous to your flock. Whether that's inviting predators or just directly not safe for birds.
I'm thinking things like having loose ropes/string hanging in the coop, ventilation issues, or even general maintenance issues.
I would love to see obvious answers as well, such as exposed nails or deep water dishes.
Feel free to add any image examples and why you think it is/could be unsafe.
 
Mishaps happen when folks think chicken wire will keep varments out of their run/coop, don't put rocks/marbles in water dishes for baby chicks, or use non-UL-approved lamp holders for heat bulbs instead of ceramic bulbs with UL-approved holders and secure with chain.

Many more things happen that wouldn't if new folks read articles in the Learning Center. That is packed with helpful and informative articles for just about everything related to poultry.

Rather than listing all the things that obviously don't belong in a chicken run, here's a thread on all the things that can be in there.
