Have you tried gently attempting to clean it and see if it's dirt? Is the chicken having any issues like odd poop, not eating, odd behavior etc?

yes, we cleaned it, it looks very bad, but the hen is behaving and eating normally, is it possible that she got hurt after fighting with another hen

Is that blood around her beak? Is the beak fractured or injured? At first look, I had thought that the eye scabbing might be fowl pox from mosquitoes, but fighting may be the cause. Has she been low in pecking order or bullied? Can you spend some time and observe their behavior? If she is injured, she may need to be separate inside a dog crate with food and water. If her beak is sore or injured, wet chicken feed mixed with water and changed often might be taken well. Is she drinking or eating?
