What is this vent issue

She had tapeworms and has been treated. Whatever this is it is making it difficult for her to poop. Picture is after soaking and getting poop off her feathers

Can you get a side view photo as well?
Is the vent "protruding"?
Almost looks like at some point she may have had a prolapse - hard to tell.

Either way - that "material" that is making it difficult for her to poop needs to come out. I don't know what it is - could be egg membrane, cecal core or something else.

If she's not lethargic - I would soak her in an epsom salts bath to help soften it, then begin working on trying to loosen and remove the material. Once you get it out, post some photos.

This may be a process of letting the hen rest (you too), soaking and working on getting the material out over the course of a day or several days. In between soaking/working on her, I would apply some type of ointment or oil to keep that moist. Coconut oil or mineral oil, an something like Prep H, etc.

Is she alert - able to eat/drink on her own?
Curiosity - what did you use to treat the tapeworms?
 
Were the tapeworms on the vent or in her poop? What did you treat them with? Sometimes they can get maggots around the vent if it is soiled with droppings. Hopefully, it wasn’t maggots or flystrike. I would give her some coconut oil orally—chill it to make it solid and cut a tsp into small slivers and offer them to her to peck. Follow @Wyorp Rock ’s advice on soaking. Use a rubber glove and lubricate your finger, and insert s finger into the vent and clean away any solid material or hardened urates. Apply some ointment on and into the vent. Repeat the soaks every day. Giving some probiotics daily for a few days would be helpful.

Not lethargic, eating fine, treated with zimectin and her poop was full of tapeworms.

Even in the photo you posted, it looks like it has bled a bit. Keep try to get it loosened up and work on. Keep the material moist. Mineral oil may be a bit better than coconut oil - try a few things to see what helps.
You may have to work it in stages and take out small pieces at a time.
