She is such a sweetie and I feel so bad for her. I want to help her! Thank you so much for the vitamin suggestions! Heading in to town right now and will get everything! How long should I do the vitamins? Week? 2 weeks? Hope to see some change!Thanks for the video, what a sweet girl!
She's very unbalanced. I would start her on Vitamins asap.
Give 400IU Vitamin E and 1/4tablet B-Complex daily along with little egg for the uptake of E. See that she's eating her normal feed and drinking well.
Deworming *might* help, but I'm not sure if worms at 5weeks old would be that big of an issue. Most folks don't even think about deworming until birds at least 6weeks or older and there's a known problem with worms in the flock.
You can find Vitamin E and B-Complex at stores like CVS, Walmart, etc. Examples are below.
IF you still wish to deworm, then you can use Safeguard 10% Liquid Goat Dewormer or Equine Paste. I'll put dosing below.

To treat most worms that poultry can have except for Tapeworm you can use Safeguard (Fenbendazole) or Valbazen (Albendazole). Both are given orally by weight; they do not mix well with water.
Safeguard 10% Liquid Goat Dewormer or 10% Equine Paste dose is 0.23ml per pound of weight given orally once a day for 5 days in a row.

Editing to add, do you see how her comb is way smaller and paler than the others? Could that also be a sign of vitamin deficiency?
