Best way to transport a sick chick to the vet?

Hello all,

I've made an appointment to take my chickadee to the vet after about a week worth of struggling and straining with pooping. Any suggestions on the best way to transport a chick to the vet with the least amount of stress? She'll be 3 weeks old in 2 days and it'll be about 55 degrees out today, so I though about using a hand warmer pouch for heat. We'll have to drive about 25 minutes to get to the vet. Should I use a cat carrier? I also have the original thick cardboard box she was mailed to me in, should I use that?

Any input is greatly appreciated.

When I have to transport young birds I use one of those small cooler bags tucked inside my shirt; comforting and warm for the bird.
