My hen Quinn is 5 years old and has been sick for the past 3 weeks. Initially, I suspected coccidiosis (so I used corrid), a stuck egg (so I bathed her in warm water to help pass her egg), and a sour crop (her crop was empty). She was isolated after she started acting lethargic about three weeks ago. She did not seem to improve, however would sometimes perk up and act normal and then a few minutes later would be hunched over and lethargic with a pale comb and wattle. In the past, I have had sick chickens, but none of them lasted 3+ weeks while sick. She has been having water with probiotics and electrolytes, but overall I am noticing a decline in her health. Also, when she was 2 she got sick, and since then she has not laid any eggs. Any advice would be greatly appreciated. I am started to get really worried and would love any guidance.

What's her diet? Has she ever been wormed?
She has not. I went to tractor supply to get a dewormer around 2 weeks ago, and an employee looked at a picture of her poop and said that she most likely did not have worms. Additionally, none of the other chickens are sick. She eats chicken feed from tractor supply with occasional mealworms and leftovers for treats

You can't tell from a casual glance at poop unless she's overloaded.
Can you get some valbzen goat wormer?
What feed?

Thanks for the feedback!! She uses DuMOR 16% Layer Pellet Poultry Feed. There is safeguard got dewormer at a store near me. However, I am scared that if I deworm her and she doesn’t have worms, she will develop immunity to the dewormer. Why do you think she has worms?

She won't develop immunity, if you use it once a month, the worms might but you're not doing that.
She "would be hunched over and lethargic with a pale comb and wattle. " those can be symptoms of not getting the nutrition shes eating. it won't hurt to worm.

By deworming her just in case you can exclude worms being the cause of her decline. It won't hurt her.

Gently feel her lower abdomen underneath her vent and between her legs, is it swollen? Hard or rather squishy like a water filled balloon?
As you said that she has not laid eggs for three years since she has been sick, I suspect her to lay internally which causes EYP egg yolk peritonitis and water belly, both putting pressure on her organs as time progresses.
