Chicken with swollen mouth and nose

My chicken is around one year old and I noticed a week or so ago the roof of this hens mouth is swollen and so is one of its nostrils. I can’t figure out what this is happening no other chickens in the flock have this issue. What should I do?

It could be canker, a protozoan disease that causes swelling and plaque lesions in the mouth. A chicken can have trouble eating and swallowing and possibly die.
